Medication In School Consent Forms
Consent Form A - Parental consent to administer prescribed medication in school
Consent Form B - Parental consent to administer Paracetamol/Ibuprofen or Anti - histamine if your child develops the relevant symptoms during the school day.
Consent Form C - Parental consent where deemed that a administration is required to allow a pupil to stay in school, the school will administer non-prescribed medicines e.g non-prescribed cream for a maximum of 48 hours.
- Please Note - All prescribed medication needs to be issued with original dispensed packaging stating the full name of the patient. All medication needs to be in date.
To consent to the use of name and images for publication, trips around the local area, participation in swimming and rebound sessions, application of sun cream and supervised/unsupervised access to the Internet.
To consent to Rebound Therapy with a signed medical note for persons with Down Syndrome.
